Cyropreservation of fertilized embryo and sperm

We just went through our 1st successful ICSI procedure and the doctor was able to freeze two high quality embryos and my husband sperm. We currently live in NY but will be moving back to Midwest. Does anyone know if the office is able to transport the frozen embroyos / sperm and the effective rate after doing so.
