What do doctors consider TTC??

Kinda a stupid question, but......................
What do doctors consider TTC?? Do they count just not using birth control, but not tracking or putting any pressure on when you have sex as also TTC? Or do you have to be having sex when you think you might be ovulating, like every other day, with your feet up in the air to be considered trying?? Just wondering. If anyone answers my question that would be great!! Thank you!!
